Where can I get a Missouri Driving Record?

To obtain a copy of your own driving record
in the state of Missouri, you will need to download the Request from Record Holder, which is found at http://dor.mo.gov/mvdl/drivers/forms/4681.pdf .

Complete the form and take it in person to the Department of Revenue contract office in your area.

If you would rather apply by mail, mail the form with a check or money order for the fee to:

The fee for a driver record is $1.25. However, if you need a certified copy, you will need to pay $4.

To request someone else’s driving record, you will do the same thing. However, unless you have that individual’s signed permission, personal information such as Social Security number and description will be removed from the report. You will need to know the individual’s name, birthday, and driver’s license number to make the request.
