To obtain a copy of your own driving record Complete the form and take it in person to the Department of Revenue contract office in your area. If you would rather apply by mail, mail the form with a check or money order for the fee to: Customer Services Division 301 West High Street, Room 360 Jefferson City, MO 65101 The fee for a driver record is $1.25. However, if you need a certified copy, you will need to pay $4. To request someone else’s driving record, you will do the same thing. However, unless you have that individual’s signed permission, personal information such as Social Security number and description will be removed from the report.
