Where can I get a Montana Driving Record?

Answer:
You can request a copy of any Montanan driving record
through the mail, in person, or online. To make the request though the mail, you will need to download the Release of Driving Record form from http://doj.mt.gov/driving/forms/34-0100.pdf .


This form must be signed and notarized or accompanied by a copy of the individual’s driver’s license. Send the form along with a self-addressed, stamped envelope and a check or money order for $4 to:

If you wish to receive the record via fax, include your fax number instead of the self-addressed envelope. This costs an additional $3.

To make the request in person, you will have to download and complete the form. Then, drop it off along with the $4 fee at:

If you are a company that needs to request driving records on a regular basis, you can apply online. This costs $50 annually, and you must register with the MVD. The cost is $6.50 for each record that you request in this manner.
