Complete the form and mail it to:

Be sure to include a self-addressed, stamped envelope with your request. The fee for the record is $3.

If you need to order several driving records for commercial purposes, you will need the Application for Copies of Several Driving Records from http://www.dmv.state.ne.us/dvr/pdf/multidrvrec.pdf .

Complete the form and mail it to the address above, along with a self-addressed, stamped envelope and $3 for each record you requested.
