One common way to get a copy of your own New York driving record is All you have to do is add $10 to the check you are sending with the renewal. If you would rather order by phone, you can do so by calling 800-225-5368 if you live in area codes 315, 518,585,607, or 716. All other residents must call 518-473-5595. There is an added $5 fee for this convenience, and you must pay via credit card. If you are not renewing your license but still need a copy of your driving record, you will need to download Form MV-15 from http://www.nydmv.state.ny.us/forms/mv15.pdf . Send it, a check or money order for $10 made out to “Commissioner of Motor Vehicles,” and a copy of your identification to: MV-15 Processing NYS Department of Motor Vehicles 6 Empire State Plaza Albany, NY 12228 If you need the copy quickly, you can get one immediately by visiting a DMV office in your area. You will need to show your identification and pay the $10 fee. If you need someone else’s record, you must have permissible use to read it or written consent from the individual.
