Where can I get a Tennessee Driving Record?

Answer:
If you would like a copy of your own Tennessee driving record,
you can make the request in person or through the mail.

To make the request in person, visit the TOS office in your area with your driver’s license.

If you wish to make the request by mail, download the Driver Data Information Request from http://www.tennessee.gov/safety/forms/dppa.pdf and complete it in its entirety.

Mail the form to:

Tennessee Department of Safety
1150 Foster Ave.
Nashville, TN 37210

Whether you apply in person or through the mail, you will pay $5. Make any checks or money orders to the Tennessee Department of Safety.

If someone else is accessing your driving record, they will need a signed and notarized statement of permission from you, unless they are in an official capacity that gives them the right to access your record.
