Where can I get a Utah Driving Record?

Answer:
You can request a copy of your Utah driving record
by visiting the DLD in person or sending in a written request. If you choose to visit in person, you will need to have your driver’s license and Social Security number.


For a non-certified copy you will pay $4.25. If you want a certified copy, you will have some extra paperwork to fill out. You will also pay $9 or more, depending on how many pages there are in the document.

If you would prefer to request a copy in the mail, you will need to download form DLD60 from http://driverlicense.utah.gov/pdf/mvr%20request.pdf .

Have your signature notarized and send the form with a check or money order for the $4.25 fee to:

P.O. Box 30560
Salt Lake City, UT 84130


Unless you have a permissible use, you can only get the driving record for someone else if you have a signed DLD60 form indicating that you have permission to access the individual’s record.
