You can request your Illinois driving record You will need to turn in the form with a $12 fee, and you will be given the copy immediately. If you would rather apply through the mail, send a written request and a check or money order for $12 to: Driver Analysis Section Secretary of State 2701 S. Dirksen Parkway Springfield, IL 62723 If you are requesting the record of someone else, do the same steps as described above. However, you will need to know the license number, date of birth, and full name of the individual.
