Where can I get an Indiana Driving Record?

To obtain a copy of your Indiana driving record,
you will need to pick up or download State Form 48430. You can download it at this link: http://www.in.gov/icpr/webfile/formsdiv/48430.pdf


Fill in the form and mail it to:

Bureau of Motor Vehicles
Driver Records/IGCN
100 North Senate Ave., Room N405
Indianapolis, IN 46204


Be sure to include a check or money order for $4 for each record you requested.

If you need to get the record quickly, visit one of the reinstatement agencies in the state. If you are requesting the driving record of someone else, you will need to use the same form. However, you will need to certify the reason for making the request.

If you do not have a legal reason, such as insurance or employment purposes, you will have to have a signed release form from the individual whose record you wish to obtain.
