Where can I get an Ohio Driving Record?

Answer:
You can request a copy of your Ohio driving record
either in person or through the mail. To make the request in person, visit the deputy registrar’s branch in your area.


You will need to fill out the Driver Abstract Request and show your ID. The cost is $5.50, and the driving record will be printed at the office.

To make the request by mail, you will need to download the Record Request form from http://www.bmv.ohio.gov/pdf_forms/1173.pdf and mail it with a check or money order for $2 made out to Treasurer, State of Ohio. Mail all of this to:

Attn: Record Request
P.O. Box 16520
Columbus, OH 43216


If you need someone else’s driving record, you will also need a copy of the Notarized Written Consent Release of Personal Information form from http://www.bmv.ohio.gov/pdf_forms/5008.pdf . This form must be signed by the individual whose record you are requesting and notarized. However, if you need the record for one of the reasons listed on the Record Request, you do not need this document.
