To obtain a copy of your own driving record, Indicate the type of record you are requesting when filling out the form. Send it with a check or money order made out to the Oregon DMV to: DMV Headquarters Attn: Record Services 1905 Lana Ave. NE Salem, OR 97314 If you need someone else’s record, you will need to send the request along with a note that indicates that you understand that the individual’s personal information will be omitted from the report. You will need to know the individual’s full name, birthday, address, and license number to access this report. The fee for a driving record varies depending on which record you are requesting.
